They have a shrine to Trina and are following her from a distance.

Spencer Cassadine Stalking Trina?
Trina and Gio Palmieri (Giovanni Mazza) were watched during their soundcheck at the Cat’s Eye, and we have yet to see the identity of the person watching them.

Both have reasons to fixate on Trina, but we love the idea that Spencer could be alive and watching Trina from afar.
If so, Quinn and Heather are red herrings, although we know that Heather will be back onscreen soon.

Interestingly enough, fans recently saw Trina reminisce about never living with Spencer.
That’s a big clue, as is Spencer’s past behavior.

We also know that the stalker erased Gio from a picture with Trina, and Spencer was always insanely jealous.

I would love to see Spencer return to Port Charles, but I’m not convinced he’s the stalker.
Spencer was jealous of Trina’s relationships in the past, but secretly watching her and building a shrine takes things to another level.
If it really is Spencer, the writers have some serious explaining to do!
Still, bringing him back from the dead would certainly shake things up for Trina and Gio.
